Headline: Conversations that Count day 2018

Thursday 5 April 2018 is Conversations that Count Day – a day to encourage people to think about, talk about and plan for their future and end-of-life care.

The day is coordinated by the Health Quality & Safety Commission.

At a meeting of DHB CEOs, 18 DHBs agreed to continue to fund the advance care planning programme nationally until the end of December 2019. The Commission will continue to coordinate the programme.

An advance care plan allows consumers to record:

  • what’s important to them
  • who they want involved in decisions about their treatment and care
  • how much treatment they want and when they want it to stop
  • where they would like to be cared for.

National support, including printed resources will be available for regions wishing to use Conversations that Count Day to promote advance care planning.
